About the Model

What is the GUIDE Model?

The Guiding an Improved Dementia Experience (GUIDE) Model is Medicare's groundbreaking initiative designed to improve dementia care through coordinated care navigation, caregiver education, respite services, and interdisciplinary collaboration.

It gives individuals living with dementia and their families access to a dedicated care navigator, 24/7 support, and connections to community resources — bringing structure and continuity to a journey that too often feels fragmented and overwhelming.

At this summit, keynote speaker Dr. Whitney Williams will walk attendees through who qualifies for GUIDE, how referrals work, and how healthcare professionals, legal advisors, and senior care specialists can collaborate within the model to better support patients and the families who care for them.
